Schedule of Benefits

HOSPITALIZATION
Geo+Plus provides hospitalization coverage for hospital room and board, intensive care, surgery, medical treatment, laboratory tests, x-rays, chemotherapy, anaesthetists fees, emergency room treatment and nursing.

OUTPATIENT
Geo+Plus provides coverage for outpatient services including medical consultations, diagnosis, pathology, radiography, laboratory, x-rays, surgery and chiropractors.

PRESCRIPTION MEDICATION
Inpatient and Outpatient prescription medication is covered.

HUMAN ORGAN/TISSUE TRANSPLANT
Geo+Plus provides up to $500,000 lifetime benefit for Heart, Heart/Lung, Lung, Kidney, Kidney/Pancreas, Liver, and Allogeneic and Autologous Bone Marrow Transplants. The procedure and related services must take place in the Managed Transplant System Network, which presently consists of 22 centers of excellence across the US.

MATERNITY AND NEWBORN BENEFITS
Maternity is covered on the same basis as any other illness, after 10 months of continuous coverage. If the Maternity is covered, the Newborn is covered from the moment of birth. Routine well-baby care for the first 14 days of life is also covered.

EMERGENCY EVACUATION
Geo+Plus provides a $25,000 lifetime benefit for Emergency Evacuation to the nearest qualified medical facility while outside the US. All evacuations must be coordinated by IMG. Emergency Evacuation Benefits terminate at age 65.

MENTAL OR NERVOUS DISORDERS
Geo+Plus includes $10,000 per Policy Year, $20,000 lifetime limit, for treatment of mental or nervous disorders, after 12 months of continuous coverage.

LOCAL AMBULANCE EXPENSE
Geo+Plus includes up to $2,500 per cause for transportation by ambulance.

WELLNESS BENEFIT
Males over age 40 and Females over age 35 are covered for up to $100 per Policy Year for a Routine Physical Examination, including Pap Smear and Mammogram, after 24 months of continuous coverage. This benefit is not subject to the Deductible or Coinsurance.



Administration and Coverage

WORLDWIDE COVERAGE
Geo+Plus provides coverage for US or Canadian citizens while residing abroad, and while in the US or Canada temporarily (less than 12 months), as well as for overseas local and third country national employees.

PROVIDER CHOICE
You may use the doctor or hospital of your choice at home or abroad. If you select a doctor or hospital from the Geo+Plus Preferred Provider Organization, your deductible and coinsurance will be waived. If you select an alternative doctor or hospital, normal benefits apply with no penalty.

DEDUCTIBLE AND COINSURANCE
You may choose between five deductible plans: $250, $500, $1,000 and $2,500. After the deductible is satisfied the Company will pay 80% of the next $5,000 of eligible medical expenses, then 100% of eligible medical expenses to $1,000,000 (internal policy limits may apply).* The deductible and coinsurance is waived if treatment is rendered by an Indy+Plus Preferred Provider. After three members of a family have satisfied their deductible during a Calendar Year, the deductible is waived for further family members. After the deductible and coinsurance has been met by three members of the same family, further claims from the family will not be subject to the deductible and coinsurance during the Calendar Year.

Eligibility, Limitations & Enrollment

ELIGIBILITY
1. Full time employees (30 hours per week), and their Spouses, at least age 19 and not yet 65 as of their initial effective date.
2. Dependent Children under the age of 19, or 23 if full time student primarily dependent on the employee for support.

LIMITATION ON U.S. COVERAGE
Initial departure from the US must occur within 30 days of the employee's initial effective date. Coverage terminates on the earliest of the following dates:
1. The date the employee ceases to be an active full time employee; or
2. The date the employee becomes eligible for any other medical benefit plan, whether or not the employee elects to become covered under such plan; or
3. Twelve months following return to the US.

PRE-EXISTING CONDITIONS
A pre-existing condition is any Injury, Illness (including Pregnancy), or Mental or Nervous Disorder, including complications and consequences associated with these conditions, for which diagnosis or treatment was received or prescription medications or drugs were taken or for which a reasonably prudent person would have sought diagnosis and/or treatment during the 90 days just prior to the employees or dependents initial effective date. Benefits for Pre-existing Conditions are covered after six months during which no treatment (including prescription medications or drugs) is recommended or received, or after twelve months of continuous coverage under the Indy+Plus plan, whichever first occurs.

TAKEOVER PROVISION
This provision allows you to implement Indy+Plus without requiring that your employees start over on waiting periods for Maternity, Mental and Nervous, Wellness and Pre-existing Condition benefits.

UNDERWRITING REQUIREMENTS
For groups of 5 or less employees, Evidence of Insurability is required for each employee. For groups of 11+ employees, Simplified Evidence of Insurability is required for each employee. The Company, at its sole discretion, may waive these requirements if claims experience is provided.

COBRA
Medical insurance continuation is available as required by U.S. law.



Term Life & AD&D Insurance

THE UNDERWRITER
Indy+Plus Term Life and AD&D Insurance is underwritten by Certain Underwriters at Lloyd's, London.

THE BENEFITS
Term Life Insurance:
You may select from the following Principal Sum plans: * Higher amounts are available, subject to full medical underwriting and approval.

Accidental Death and Dismemberment

Accidental Loss of Life Principal Sum
Accidental Loss of Two Members Principal Sum
Accidental Loss of One Member 50% of Principal Sum



Daily Disability and Indemnity

Daily Disability and Indemnity coverage pays US$ 100 per day directly to the employee for each overnight stay in the hospital covered under the Comprehensive Medical Insurance. This optional plan provides coverage for incidental expenses and income supplement for each night the employee is in the hospital, except for hospitalizations related to pregnancy.

ELIGIBILITY
If you are eligible for Geo+Plus Medical Benefits, you are eligible for Geo+Plus Term Life and AD&D insurance and Daily Indemnity Insurance. The only exceptions are:
1. All benefits reduce 50% at age 65 and terminate at age 70 or retirement, whichever first occurs.
2. Benefits do not become effective until the employee initially departs the US.

UNDERWRITING REQUIREMENTS
The requirements are the same as the requirements for Geo+Plus Medical Benefits. Term Life and AD&D Insurance is required for groups of 20 or less employees.
